All physicals MUST BE UPLOADED ONTO RankOne before the start of the first practice.  You must print the paper physical form for your doctor to fill out, then upload it to the theRankOne site.  Both links are located to the right of this page, as well as instructions on how to sign up and use RankOne.  

Please note:  The page with Doctor's signature/stamp, date, and box checked “eligible for all activities” needs to be uploaded with the front page.
